Ok, I haven't read all the post above, been away for some days.

It is strange what problems you have with MyIDE-][
Normally it all goes like a charm, I'm very puzzled....
Do not use the 128Mb card, it's to small.

Perhaps you should re-init too, on the Atari (MyBIOS).

On the ATARI, Boot, use OPTION+SELECT+START.
INIT, AUTO,
Make one partition, D1:, 65535, D, "big-disk", save&exit.
Perhaps you want to increase partition-space with INIT/IMAGES, move +1, to have about 64Mb more room for all partitions later.
So total 128Mb for partitions, the rest is for images.

Move to the PC.
Format the CF-card with FAT32, check the size, it should be about 40% of the original size stated on the card.
(Since that is reserved for PC-FAT32, the other 60% or so is reserved for MyBIOS).
Start MyIDE-manager_lba2, it should find your MyBIOS-card.
Click on Partitions.
Copy a 16Mb disk over, safely remove the hardware and then eject.

Back to the ATARI, now, does this boot.......? It should, since it's drive #1.....
