Easy...... So close.....

Workaround:
First make D3 as SD "bootpart", not D1
(Go back to the menu and change partition A to drive 3.)
Save/exit
Boot from aspect
Format/Write DOS to D3:
Go back to the menu
Change partition A to drive 1
Save/Exit
Done.

Color-bars:
System is powered-up but had nothing to boot.
XL/XE would send you to selftest, 400/800 to MEMOPAD, I give nice colors :-)

BIOS is active, so press SHIFT+CONTROL+TAB to reboot or do something else.

----

So yeah, your card is fine, boot priorities wrong and you did not know what the colors mean.
You can not make a partition called D1: and simply save and exit.
There is nothing to boot from (yet).

Later,
Sijmen.

[EDIT]
You must start with a unassigned drive when you first init.

From the manual:
• Press M (Make), <enter> (unassigned drive #), 720 <enter> (Sectors #), S (density). Name: BOOTPART (example)
This is your primary boot partition for system-related software.
There is not a drive-ID attached to it, since we can’t boot from it yet.
